Parents/Guardians will be asked to complete both of these forms for all youth programs, classes or camps. Download these forms and bring them to your first day of class or plan to complete them on the first day of class in the classroom. Your instructor will have spare copies.
This form contains important information such as emergency contact, who may pick up your child from class and information regarding medications and allergies. If a child in class has serious allergies or health concerns, you will be contacted via email notifying you which foods and/or items may not be allowed into class. This helps us create a safe and allergen-free environment for all young artists who join us at Mesa Arts Center!
This form includes 2 permission forms stating whether Mesa Arts Center is allowed to take photos of your child, and if your child is allowed to leave unattended at the end of the class. If your child is not allowed to leave unattended, you acknowledge that as the parent/guardian that you will or someone else will pick them up at the classroom at the end of each class.
